NOTE: This form is ONLY for customer returns related to Philips Lighting Electronics ballasts under the warranty period and for U.S. ship to locations (no Canadian or non-U.S. ship to locations). For warranty information on all other locations outside the U.S., refer to our main warranty page for specific warranty information.


If you are a distributor or agent and want to return good ballasts or inoperable units, please call customer service at: 1-800-372-3331 and select option #1.


Using this online form will allow us to process your request more efficiently, but if you would like a paper copy please use the following form. You can even type your information directly onto the form then FAX it to Philips Lighting Electronics N.A.


THIS FORM CANNOT BE USED TO REQUEST THAT MATERIAL BE RETURNED FOR CREDIT TO YOUR CORPORATE ACCOUNT. IF YOU NEED TO RETURN MATERIAL FOR CREDIT THEN PLEASE CALL US AT 1-800-372-3331 AND CHOOSE CUSTOMER SERVICE OPTION #1.
